Registration Documents

If your student has been offered enrollment at Salt Lake Academy through our randomized lottery process, you will be sent an acceptance email. The next step is to register your student using the information provided in the acceptance email.

Using the form below, please upload your student's birth certificate and immunization records, or immunization exemption form from the Health Department, current transcripts if enrolling in grades 9-12, and a copy of your student's IEP and/or 504 accommodations if applicable.

State law requires that we are presented with the original documents.  Please bring this to the High School office at your earliest convenience. Please check our School Hours.
